35 years old gentleman progressive difficulty in walking and impaired dorsal column function initially diagnosed as C3-6 Intramedullary cervical Tumor and refused surgical treatment 2 years ago. Patient complained of recent worsening of symptoms and further extension of Tumor C2-T2 and Syrinx formation till T9 level along with oedema and complete loss of dorsal column function, myelopathic gait and breathing difficulty along with chest tightness. Gross total excision of Tumor achieved by C3-T1 Enbloc Laminotomy and excision of Tumor. Surgery took 14 hours and uneventful operative and postoperative course. Intramedullary ependymoma are the most frequent intramedullary tumors in the adult population. The gold standard treatment is to obtain gross total removal without any new postoperative neurological deficit and we achieved it.
